What usually causes neck pain
Neck pain is very common and, in most cases, not a sign of anything serious. It can come on after a long day at a screen, an awkward night's sleep, a sudden movement or for no obvious reason at all. Most episodes settle within a few weeks, and the right advice and exercise can speed that up and make it less likely to keep coming back.
Sometimes neck pain spreads into the shoulder blade or down the arm, brings on headaches, or comes with pins and needles in the hand. Working out which of these is going on is the first job, because each needs a slightly different approach.
Neck problems treated at home
Stiff, painful neck
The most common type. Pain and stiffness that is worse with certain positions and eases with movement. Usually settles well with graded exercise.
Headaches from the neck
Headaches that start at the base of the skull and come with a stiff neck. Treating the neck often reduces how often they happen.
Trapped nerve
Pain, pins and needles or numbness down one arm from an irritated nerve in the neck. Most cases improve over weeks to a few months.
Whiplash
Neck pain after a car accident or a jolt. Keeping moving, within comfort, is better than a collar or rest.
Desk and posture related pain
Pain linked to long periods in one position. Changing position often matters more than finding one perfect posture.
Neck pain in older adults
Often linked to wear in the neck joints. Stiffness responds well to regular movement and strengthening.
What helps in the first few weeks
- Keep moving. Gentle movement within comfort helps more than holding the neck still. Collars are rarely helpful.
- Change position often. At a desk or on the sofa, the best posture is usually the next one. Short, frequent breaks help.
- Heat and pain relief. A warm pack and pain relief as advised by your pharmacist or GP can make movement easier.
- Check your pillow. One that keeps your head level with your body, rather than propped up or dropped down, suits most people.
Get urgent medical help for neck pain with a sudden severe headache, worsening weakness in the arms, problems with walking, balance or clumsy hands, after a significant fall or accident, or with fever or unexplained weight loss.
Neck pain and your shoulder
Pain around the shoulder blade or down the upper arm can come from the neck rather than the shoulder, and the two are easy to confuse. - A proper history: how it started, what makes it better and worse, and whether there are arm symptoms or headaches.
- Assessment: neck movement, the nerves to the arms where needed, the upper back and the shoulders.
- A clear explanation: what is most likely going on, in plain terms.
- Hands-on treatment such as joint mobilisation and soft tissue work, where it is likely to help.
- A short exercise plan that fits into your day, and advice on work, sleep and driving.

Do I need an X-ray or a scan?
Usually not. Most neck pain is diagnosed from the history and examination, and scans often show age-related changes that are not causing any symptoms. Imaging is useful when it would change the plan, and Hamoodi will say if he thinks you need it.
How long will my neck pain last?
Most episodes settle within a few weeks. Pain from a trapped nerve can take longer, often weeks to a few months, but usually improves steadily.
Will hands-on treatment fix it?
Hands-on treatment can ease pain and stiffness in the short term. It works best alongside exercise, which is what makes the change last.
Can neck problems cause headaches?
Yes. Headaches that start at the base of the skull and come with neck stiffness often improve when the neck is treated. A sudden, severe or unusual headache needs urgent medical attention.
I work from home. Is my set-up the problem?
It can contribute, but it is rarely the whole story. Changing position regularly and building strength usually matter more than finding one perfect chair or screen height.
