Musculoskeletal Rehabilitation at home in Northampton

Most musculoskeletal problems do not need a scan, an injection or an operation. They need a clear diagnosis and the right loading programme, started before the joint has stiffened and the muscles around it have weakened. What the first visit involves

  • A proper conversation about how the problem behaves through the day, what set it off, and what you have stopped doing because of it
  • Assessment of the joint's movement and strength, plus the joints above and below it, because a knee problem is often a hip or ankle problem in disguise
  • A look at the tasks that are actually hard for you, done in the room where you do them
  • A short, specific exercise programme set at the level the joint currently tolerates
  • A straight answer about what is going on, how long it should take, and whether anything needs a GP, a scan or a consultant

Treatment and how long it takes

Most musculoskeletal rehabilitation is progressive loading: the joint is asked to do a little more each week than it did the week before, at a dose it can recover from. Alongside that, treatment usually includes hands-on work where it helps, advice on pacing and on the movements that keep aggravating things, and practice of the tasks you want back, whether that is the stairs, the garden or getting down to the floor and up again.

Many joint problems improve meaningfully over six to twelve weeks. Frozen shoulders and long-standing tendon problems are slower and are honestly measured in months. Appointments usually start weekly or fortnightly and spread out as the programme takes hold. You are not asked to commit to a block of sessions before anyone knows what is wrong.

Do I need a referral or a scan first?

No. You can book directly, and most joint and muscle problems are diagnosed from your history and a physical assessment rather than a scan. Imaging matters when it would change the treatment; if Vidhi thinks you need it, or a GP or consultant opinion, she will tell you.

Is exercise safe if I have arthritis?

Yes, and it is the mainstay of treatment for osteoarthritis in the knee, hip and hand. Strengthening does not wear a joint out. What tends to make arthritis worse is the gradual reduction in activity that pain causes, because the muscles supporting the joint weaken and everything becomes harder.

My fracture has healed. Why does it still not work properly?

Because healing the bone and getting the limb working again are two different jobs. Six weeks in a cast or sling leaves a joint stiff and the muscles around it weakened, and that does not always recover on its own. A short programme usually makes a large difference.

I am older. Is this kind of physiotherapy still worth it?

Very much so. Much of Vidhi's work is with people in their seventies, eighties and nineties, and the aim is practical: stairs, walking distance, getting up from a chair, keeping the hands useful. Progress is measured in those things, not in gym numbers.

How many appointments will I need?

Most people are seen every one to two weeks at first, then less often. Vidhi will give you an honest estimate after the first assessment rather than committing you to anything up front.
