DBS checks for self-employed physiotherapists: how to get one, and how to keep it current

Every care home, case manager and worried relative will ask whether you are DBS checked. As an employee the answer was easy: HR did it. As a self-employed physiotherapist the process is different, the rules are widely misunderstood, and getting it right early saves months of awkward emails.

The three levels, briefly

• Basic. Shows unspent convictions and conditional cautions. Anyone can apply for their own Basic check online through GOV.UK.
• Standard. Adds spent convictions and cautions (subject to filtering). Only available for certain roles and only through a registered body.
• Enhanced. Standard plus any information the local police consider relevant, and, where the role involves regulated activity, a check of the children's and adults' barred lists. This is the level care homes, case managers and NHS contracts expect from a physiotherapist, because treating adults at risk in their own homes is regulated activity.

Why you cannot just apply for an Enhanced check yourself

Standard and Enhanced applications have to be countersigned by a registered body that has confirmed the role is eligible. An individual cannot countersign their own application, so a self-employed physio with no employer has historically been stuck: eligible for the check by the nature of the work, but with nobody to submit it.

The route around this is a DBS umbrella body: an organisation registered with the DBS that submits checks on behalf of smaller organisations and self-employed people whose roles are eligible. You pay the DBS fee plus the umbrella body's administration charge, you provide identity documents (usually verified by video or in person), and the certificate is issued to you. Search GOV.UK's list of responsible organisations and choose one that explicitly handles self-employed healthcare professionals. Expect the whole thing to cost roughly £60 to £90 and take two to six weeks, longer if the police information stage is slow in your area.

The rules on eligibility for self-employed applicants have been tightened and clarified in recent years, and umbrella bodies are careful about it. Be accurate about your role (physiotherapist providing treatment to adults, and children if you treat them, in their own homes and in care settings) and the umbrella body will tell you which workforce and which barred list checks apply. Do not overstate the role to get a fuller check; that is an offence.

Join the Update Service on the same day

A DBS certificate is a snapshot on the day it is printed. Nothing on it updates, and a referrer looking at a certificate from three years ago is entitled to wonder what has happened since. The DBS Update Service fixes this. For a modest annual subscription, your certificate is kept live: anyone you give permission to can check online, instantly, that nothing has changed since it was issued. Two rules make it work:

• You must register within 30 days of the certificate being issued (or during the application, using the application reference). Miss the window and you need a new check to join.
• Keep the subscription running. Let it lapse and the certificate reverts to a snapshot.

For a self-employed physio this is not optional in practice. It means you never pay for another check for as long as the subscription runs, every agency you locum for can verify you instantly, and you can answer a care home's compliance question with a status check rather than a scan of a document.

What to send referrers (and what not to)

Care homes, case management companies and solicitors will ask for evidence. The right answer is: your certificate number, the date of issue, the level, and your consent for them to run an Update Service status check. Do not email scans of the certificate around. The certificate contains sensitive personal data, and once it is in someone's inbox you have no control over it. A status check gives them the assurance they need and keeps the document with you. Physios who treat children

If your caseload includes children (paediatric neuro, developmental work, school visits) your check needs the children's workforce and the children's barred list as well as the adults'. Say so when you apply. An adults-only check will not satisfy a school or a paediatric case manager, and you cannot add a barred list to an existing certificate; it means a new application.

Scotland and Northern Ireland

The DBS covers England and Wales. In Scotland the equivalent is the Protecting Vulnerable Groups (PVG) scheme run by Disclosure Scotland, and self-employed practitioners join it through a similar registered-body route. In Northern Ireland it is AccessNI, and Enhanced checks likewise go through a registered umbrella body. If you work across a border, you may need both.

Where DBS fits in the launch sequence

Start the application in the same week you sort your insurance, because it is the slowest item on the list and everything else will be ready before it. Join the Update Service the day the certificate arrives. Put the certificate number and issue date in the same folder as your insurance schedule and your HCPC registration, and you have the three things every referrer asks for, ready to send from your phone. Then say it everywhere: "HCPC registered, fully insured, Enhanced DBS checked" is the phrase that makes a daughter stop scrolling.
