Postural management is about how someone is supported through the whole day and night, not just during a therapy session. For children and adults with neurological conditions, complex physical presentations or reduced mobility, getting that right can make a real difference to comfort, function and long-term posture.
What postural management actually means
It is the individualised assessment and management of posture, positioning and movement. Rather than looking at one position in isolation, it considers lying, sitting, standing and moving as a single continuous picture, because posture is shaped by every hour of the day and night.
The aim is to support comfort, function and participation, and to reduce the risk of postural change over time.
What an assessment covers
A postural management assessment at home typically includes:
- Assessment of posture and positioning across different environments, including lying, sitting, standing and mobility
- Identification of factors contributing to postural changes, such as asymmetry, discomfort, reduced function or an increased risk of contractures
- Individualised positioning programmes and advice for families, carers and support staff
- Advice around standing and positioning equipment, including sleep systems, wheelchairs and standing frames
- Advice on night time positioning, including strategies and appropriate equipment where required
- Regular review and adjustment as needs, posture, function or equipment change
Why the 24 hour picture matters
Someone with reduced mobility may spend most of the day and night in a small number of positions. If those positions are asymmetrical or unsupported, the effect builds up over time. A programme that works well in a wheelchair during the day can quite easily be undone overnight.
Looking at the full 24 hours makes it possible to see where the pressure is actually coming from, and to build a plan that fits around a real routine rather than an ideal one.
Lycra orthoses
Dynamic Lycra orthoses are close-fitting garments that apply gentle directional support to help align posture and improve control of movement. They can form part of a wider postural management programme. Claire is DM Orthotics certified, so assessment and provision can be arranged as part of a plan.
Working with families, schools and case managers
Postural management usually involves several people around one person: family, carers, school or day service staff, wheelchair services and equipment suppliers. Recommendations only work when everyone is doing the same thing, so a good programme is practical, clearly explained and reviewed as things change.
Claire provides detailed written assessments and reports, attends meetings in person or virtually, and works alongside case managers arranging support for a client.
