Falls prevention balance and strength training for older adults

Falls are the leading cause of injury in older adults across the UK, and West Yorkshire and North Yorkshire are no exception. The good news: targeted home physiotherapy can meaningfully reduce your falls risk — evidence shows that balance and strength training can cut fall rates by up to a third.

Understanding the risk factors

Falls are rarely caused by one thing. A comprehensive assessment looks at the combination of factors contributing to your risk. Typical factors include:

  • Muscle weakness: Especially in the legs and around the hips. Loss of strength is one of the biggest modifiable risk factors.
  • Poor balance: Both static balance (standing still) and dynamic balance (moving around).
  • Reduced mobility: Slower walking, shorter steps, and hesitation when walking.
  • Vision changes: Poor eyesight affects how you judge distances and obstacles.
  • Medications: Some medications cause dizziness or drops in blood pressure.
  • Home environment: Loose rugs, poor lighting, clutter, and lack of grab rails.
  • Foot problems: Pain, poor footwear, or foot deformities.
  • Fear of falling: Often creates a vicious cycle of reduced activity, further deconditioning, and increased risk.

Assess your home environment

I look at access, stairs, rugs, furniture layout, bathroom safety, lighting, and potential trip hazards. Your home is unique — and so are the hazards within it.

Measure your strength and balance

Using validated clinical tests (such as the Timed Up and Go, Tinetti Balance, Berg Balance Scale, and 30-second sit-to-stand), I measure your current strength and balance to identify specific areas to work on.

Design a tailored exercise programme

Based on the assessment, I create an individualised exercise programme targeting the specific weaknesses and balance deficits I've found. All exercises are evidence-based and aligned with NICE guidelines and the Otago / FaME programme principles.

Practise real-world tasks

We work on getting in and out of your own chair, using your own stairs, walking in your own hallway, and managing your own kitchen. This transfers directly to daily life in a way that clinic-based rehabilitation cannot.

Recommend equipment and adaptations

Where appropriate, I recommend walking aids, grab rails, raised toilet seats, or non-slip mats — based on your home and needs.

Build confidence

Many older adults limit activity through fear of falling. Structured exercise and education can rebuild confidence, breaking the cycle of deconditioning.

Evidence-based exercise

Exercise is the single most effective intervention for falls prevention. NICE guidelines recommend structured exercise programmes focused on:

  • Lower limb strengthening: Sit-to-stand, step-ups, and resisted exercises.
  • Static balance: Standing on one leg, tandem stance, narrow-base standing.
  • Dynamic balance: Walking while turning, stepping over obstacles, figure-8 walking.
  • Functional tasks: Stair practice, getting up from the floor, reaching and bending.

Programmes typically need 50+ hours of exercise over 12 weeks or more to have the greatest effect on falls.
